Distribution of IRS 1095-C forms

IRS 1095-C forms were sent via email on Monday, March 1, and Tuesday, March 2, to all full-time and part-time employees who were enrolled in their benefits program during 2020. Please check your junk folder for the email as it appears the communication was delivered to that folder.  The email came from the address “Online Form Retrieval” with the subject “IMPORTANT TAX DOCUMENT AVAILABLE” and contains a unique secure link for each employee as well as instructions to confirm identity. Your form can then be downloaded from the e-mail.

Please note that a paper copy of your 1095-C form will also be mailed to your home address. If you would like to access the electronic version and it was inadvertently deleted from your junk email folder, please send a request to benefitsoffice@fnal.gov. You will then receive a new e-mail that will allow you to retrieve your 1095-C form electronically.
