FRA employees will receive a FermiWorks inbox item on Monday, October 25, about submitting proof of COVID-19 vaccination. After receiving the email from FermiWorks, you can securely upload your vaccination card that will be reviewed by the Fermilab Occupational Medicine Office (FOMO).
As a reminder, all FRA employees must be fully vaccinated against COVID-19 no later than Dec. 8, 2021, regardless of work location or telework status, unless you have requested and received an approved accommodation. Fully vaccinated means completing all required doses and completing a two-week waiting period.
If you previously provided proof of vaccination to the FOMO, you need to resubmit the information in FermiWorks.
